Patient Registration

As a new patient at Cimarron Women's Clinic, we ask that you fill out our Patient Registration and Health History forms. For your convenience and in order to expedite processing at the time of your appointment we have made these forms available here online (see below).

Please print and complete the forms and arrive 15 minutes prior to your scheduled appointment time with your completed paperwork and your insurance card.

(This form must be printed and filled. Also, please note that there are two sides to the Health History form which must be completed.)

Make sure you fill out our Online Registration Form if you aren't already registered with us.

Once you've registered use our Appointment Form to schedule an appointment.
